飲食與飲水限制會(huì)增加機(jī)體對(duì)抗菌藥物耐藥病原體的易感性

在醫(yī)院環(huán)境中,脫水與營(yíng)養(yǎng)不良問題十分常見,且往往存在診斷不足的情況。多重耐藥菌感染每年造成超過 3.5 萬名院內(nèi)患者死亡。飲食與限水干預(yù)(DWR)對(duì)機(jī)體易感多重耐藥病原體的影響尚不明確。本研究發(fā)現(xiàn),飲食與限水會(huì)顯著增加機(jī)體對(duì) ESKAPE 耐藥致病菌全身感染的易感性。
本研究采用耐甲氧西林金黃色葡萄球菌感染的小鼠血流感染模型,證實(shí)飲食與限水會(huì)顯著提升小鼠的死亡率與發(fā)病程度。該干預(yù)會(huì)導(dǎo)致細(xì)菌載量升高、組織嚴(yán)重病變,同時(shí)腎臟內(nèi)吞噬細(xì)胞數(shù)量增多。飲食與限水可改變吞噬細(xì)胞的功能,使其無法有效控制感染。
機(jī)制層面研究表明,飲食與限水會(huì)損害巨噬細(xì)胞對(duì)多種致病菌的吞噬能力,以及對(duì)凋亡中性粒細(xì)胞的胞葬作用。綜上,本研究凸顯了飲食與機(jī)體水合狀態(tài)在抵御感染過程中發(fā)揮的關(guān)鍵作用。
英文摘要:
Dehydration and malnutrition are common and often underdiagnosed in hospitalsettings. Multidrug-resistant bacterial infections result in more than 35,000 deatha year in nosocomial patients. The effect of temporal dietary and water restriction(DWR) on susceptibility to multidrug-resistant pathogens is unknown. We reportthat DWR markedly increased susceptibility to systemic infection by ESKAPEpathogens. Using a murine bloodstream model of methicillin-resistantStaphylococcus aureus infection, we show that DWR leads to significantly increasemortality and morbidity. DWR causes increased bacterial burden, severe patholog:and increased numbers of phagocytes in the kidney. DWR appears to alter thefunctionality of these phagocytes and is therefore unable to control infection.Mechanistically, we show that DWR impairs the ability of macrophages to phago-cytose multiple bacterial pathogens and efferocytose apoptotic neutrophils.Together, this work highlights the crucial impact that diet and hydration play inprotecting against infection.
